    As Case Manager Supervisor, your broad responsibilities will include overseeing a service delivery team while providing medical case management services to Medicare beneficiaries. Ideal candidates will possess both a nursing leadership background that enables them to supervise, coach, and easily interact with nurse care managers, as well as a professional background that enables them to easily interact and establish deep professional relationships with primary care physicians at a high level. As Case Manager Supervisor, you will collaborate with the Unit Manager to ensure consistent delivery of case management services across Maryland. This opportunity is available in Maryland, N. Virginia, and DC surrounding area.



    • Work independently in your home office setting while supervising a team of case managers.
    • Through application of a unique mix of experience and certification, your team and you will develop and assess care coordination services to improve health, reduce health risk, and prevent unnecessary hospitalizations for Medicare beneficiaries with a designated Primary Care Practice (PCP).
    • Use motivational interviewing techniques to identify patient’s readiness to change, implement appropriate actions based on assessment support the member in developing a specific action plan when ready.
    • Identify and discuss management of co-morbid conditions with patient and treating Primary Care Practice to ensure excellent risk factor control.
    • Review patient current medical status and revise care plan as needed in conjunction with patient, treating physician, and healthcare team.
    • Assess and monitor the medical regimen and patient’s response; assist the patient in developing specific care plans, and serve as the multi-disciplinary team liaison.
    • Provide medical, care management expertise, and serve as a critical communication link for the patients and families with the physicians and staff of a designated Practice.
    • Ensure timely interventions and facilitate continuous communication within the CM team and PCP while supporting patient choices with education and support.
    • Document and maintain accurate database to track patient progress and improving goals.


    • Unrestricted State Registered Nursing License in Maryland (compact state). Nursing Diploma, BSN preferred
    • 5-10 years related clinical experience in general medical, geriatrics and/or comparable field 
    • 5+ years program and staff management
    • 3-5 years preferred experience within a Primary Care office or clinic
    • National certification in related field or obtained within 12 months of date of hire
    • Demonstrated ability to work independently and along with others
    • Attention to detail, timetables, and commitment to completing tasks
    • Experience with Microsoft Windows and computer savvy
    • Must be well organized, efficient, make sound judgements, and able to prioritize and work with competing priorities
    • Ability to define, resolve problems and challenges in a workplace setting
    • Responsible for having reliable High-Speed Cable or Fiber Optic Internet service
    • Must have Excellent People Skills and Communication Skills via Phone, E-Mail, Text, Written and Verbal format, and provide 24 hour follow-up to all communication
    • 30% travel in Maryland
